I downloaded the wellness check form. I go to my doctor for annual checkup next month. He usually does everything on the form. What do I need to do when Im there and with the form afterwards?

I have annual checkups like this every ear but never got credit for it what am I doing wrong?

Does BCBS Texas cover annual mammogram, done density every 2 years and 3D mammogram? What is the cost at MDAnderson? I need to have these tests done but on limited income.
