My BCBSTX coverage includes hearing aids Costco but claim denied

My BCBSTX coverage includes hearing aids and I have a hearing deficiency. I recently had an an exam at my Costco, which was no charge, and was fitted with hearing aids much less than the cost of other providers. I submitted my claim to BCBSTX with receipts but was denied and the reason was not explained or mailed to me. Can you explain how to complete and code the forms so my claim is approved? Thanks.

And a not helpful response: "Call BCBS". Spent two hours on the phone yesterday and got nothing helpful.
