My wife is having her annual wellness exam. How do I get the form for the doctor to fill out so I can get the discount next year?

Hello, We do not have a form for this service. The provider would submit the claim to us the same as any regular claim. You may wish to verify with your employer's benefit department if there is anything particular they need to ensure the discount. ~ Kayla
