Why can BCBS TX NOT get direct Debit set up correctly?

Very disappointed to twice be threatened wither withbeing cut-off or worse having to reapply to get reestablished due to a failure on BCBS part to get the approved ACH to work. We have just spent 30 minutes being told by CS that they cannot tell if we did nor did not pay April, or May, but yes the information is in the correct fields to create a debit. 

This occurred in Feb, worked for March, April, Failed in May. No explanation or anyone can we talk to, no supoervisor to be escalated to, no cpontact in the finance department, no supervisor that we can call back in to discuss our concerns. Pretty tough service level at $18,000 a year.
