Who can I contact to make sure my daughter is covered as well and that my coverage is "Family" not "Single"?

I previously removed my daughter from my coverage plan, but re-enrolled her during the open enrollment time in November. I am getting charged on my pay stub for covering her insurance, but cannot tell if she really is covered or not. I would like to know before I take her to the doctor! She lost her Medicaid coverage this past year and I would like to know if she is covered or not  and what I can do to change that if that isn't currently updated in my BCBS account.
