mammogram

I want to have a mammogram. My plan is Blue Choice PPO. I am 39. I want to know if I need a doctor's order for a mammogram ? Can I schedule an appointment with in-network providers who perform a mammogram ? And if i choose in-network providers, are all services covered ? Will it incur any charges on me ?
