How do I request a Loss of Coverage Letter for my dependent who turns 26 next month?

My dependent turns 26 next month and will automatically lose coverage under my plan. Her new insurance that she is applying for is asking for a Loss of Coverage Letter to verify this fact. How do I get this sent?
