My husband already has a cardiologist but we are changing doctors to get one in the network. Does he have to get a referral from the current doctor or from our primary care doctor?

    Hello, There is not a form needed to change providers. If your plan requires that you select a PCP we can help you make that change. If your plan does not require you to declare a PCP we would not need to make any changes in our system as we would not track your doctor's name. If you'd like to send us a private message with your plan ID and contact information our outreach team can check the network status of this doctor and can check to see if your plan requires you to select a PCP and if so they can update this information in our system for you. We do recommend deleting the post with your email address to help keep your information safe. ~ Kayla
