Need a Document Uploaded to my Application, Customer Service Says They Can't See It

Is there any way to upload this document to my application? Customer service representatives keep saying they cannot see this document. I uploaded it on the HealthSherpa page where I originally began applying on 11-30-24. They keep telling me they can't see it. They have also asked me  to fax it, but of course I don't have a fax machine. Is there any possible way someone can add this to my health insurance application so that I can be covered?
