HOW DO I FILE A COMPLAINT FOR PROVIDER SERVICES ABOUT A CUSTOMER SERVICE REP THAT IS RUDE OR EITHER INTENTIONALLY DISCONNECTS PHONE CALLS?

I would like to know how I start on filing a complaint for your customer service rep? I always have issues when contacting your customer service thru the provider line. They either hang up on you or be rude.

I spoke today with a provider customer service rep named Adam with ref # 92712357 who is extremely rude. I am not sure what is going on with your customer service but you guys have the worst customer service ever!
